There are a tremendous number of duplications in the character's
names, their appearance, their manner of death, their professions,
and in the circumstances in which they find themselves.
These
duplications are the result of the 'game players' choosing
characteristics and scenarios from a pre-set list of choices
or options. For example, these choices may result in characters
that utilize the same avatars so they look the same, or with
characters having the same name, or with an unusual number
of people dying from a broken neck.
Once
again, it is possible that the game itself may be inserting
some of these, as is the case with the duplicate Fail Safes.
The game itself obviously required that the Fail Safe in each
of Levels 1 through 5 that had to be identified and utilized
for the 'game players' to move on to the next level of the
game.

have been two verbal references to a Fail Safe. Not only did
Desmond have and use the Fail Safe key in S2E24, but if you
go back and watch S1E24 you will notice that when Locke is
talking with Jack, he refers to splitting up the dynamite
as a Fail Safe. For the importance of there being two different
Fail Safes see the Level 1 and Level 2 Objectives in Playing
and also see the individual Levels for the use of the Fail
Safes in each level.

1 = Dynamite is split up before being used to blow open the
Swan Hatch
Level 2 = The Fail Safe Key is used by Desmond
Level 3 = Juliet entered the numbers into the key pad at the
sonic fence
Level 4 = Daniel enters numbers into the computer at the Tempest
to stop the gas from being released
